The main aim of the nutrition department is to treat underweight and overweight in children and adults, and the pathologies correlated to it, like hypertension, diabetes and dyslipidemia. The nutrition department can treat these problems with the help of customized diets that consider the patient needs and pathologies. The purpose of this service it to teach to our patients to choose a correct nutrition during the entire life (pregnancy, childhood, during the sport activity).
The doctor’s role is to teach to the patient what are the best diet, behavioral, phytotherapic and pharmacological strategies to lose weight.

The prescription of a therapy for the treatment of obesity is preceded by the analysis of the patient body composition (portion of fat mass, non-fat mass and body water), with the help of the caliper and the impedance analysis.
The skinfold test is non-invasive and easy to perform. It consists in the measurement of the body circumferences and of the subcutaneous fat layer. With the help of some elaborated software, the doctor can calculate the percentage of fat and non-fat mass. The impedance analysis is quick and non-invasive: thank to the passage through the body of low voltage electrical current, that the patient does not perceive, the doctor can calculate the percentage of body water fat-mass and non-fat mass.

The first consultation
During the first consultation, the doctor evaluates the patient medical history and his/her food habits. During the consult, it is essential for the doctor to identify eating disorders like anorexia, bulimia, compulsive eating disorders, that will require the support of a psychologist. If necessary, the dietologist can prescribe some bloodwork, in order to exclude some factors that can be responsible for weight increase or decrease. At the end of the consult, if the patient is not affected by serious pathologies and if the patient has not particular requests, the doctor can prescribe a diet and plan some physical activity.
For example, as per the weight loss, the aim is to lose the 20% of the body weight in one year. Anyway, a reduction of 10% of the fat-mass is considered enough to prevent pathologies related to obesity. The maintenance of weight is another essential aspect of the therapy. The aim established by the guidelines, is to maintain the weight loss for 5 years after the diet. Unfortunately, around the 80% of patients can maintain their weight after 5 years. The recovery of the weight usually happens because the patient leaves the suggestions and the lifestyle provided by the doctor.
The dietologists can also handle with the childhood obesity, collaborating strictly with the family and with the help of a food diary filled in by the patient or by the family. Diets, supplementary strategies and physical activity
According to the type of patient, the doctor can associate to the diet some behavioral, phytotherapic and pharmacological strategies. It is important to combine the diet with some physical therapy (prescribed by the doctor according to the patient health conditions). The patient can perform physical activity in MediClinic: we are equipped with a modern gym and pool to perform pool therapy. For example, the program for a cardiopathic patient is thought to be different for intensity and for type of work from the program for a diabetic patient. All the programs are prepared following the international guidelines. In the past the physical therapist suggested a specific type of physical therapy; nowadays the researches show that each patient needs a different program, depending on the type of pathology.
The physical therapy prescription in a cardiopathic or in a diabetic patient is preceded by a stress test, performed by the patient according to the international guidelines.
Obese patients that are not affected by severe pathologies should perform 250 minutes per week of physical therapy in order to lose weight. To maintain the weight loss, patients should perform 180 minutes per week of physical activity. Patient should conduct physical activity from 3 to 5 sessions per week, and each session should last at least 60 minutes. It is recommended to conduct aerobic activity, that allows to the patient to reach the 70-75% of the maximum cardiac frequency for his/her age; the sport activity should involve the major muscle groups of the lower limbs. If necessary, the patient can associate to the aerobic activity the anaerobic activity, in order to strengthen the muscles. After the first consult, the monthly follow up can highlight the weight loss and, if necessary, re-evaluate the diet, according to seasonality, patient needs, paraphysiological conditions as pregnancy.
